Our repeat medication request form is for pets who are on long-term medication.

If your pet has a new problem or a reoccurring issue which they are not already on medication for we won’t be able to prescribe medication without a consultation. In this case please book an appointment rather than using this form.

Prescribing medication is subject to many regulations which means we may have to see your pet before we are able to prescribe.

The maximum amount we will prescribe at one time is 12 months of flea or wormer medications and 2 months of most other long-term medications. In some cases the vet may decide only to prescribe less.

Please allow 7 working days for medication requests to be processed.

We offer collection from your chosen Animal Trust surgery or postal service for a £5.50 supplement. Please note that refrigerated items and scheduled pharmaceuticals must be collected and cannot be posted.
