Clinical History Upload

When a pet transfers to Animal Trust from another veterinary practice, it is essential that we receive their full and up-to-date clinical history to ensure safe and continuous care. Without this information, we may be unable to provide the level of care your pet requires.

This form enables the previous practice - or the pet owner, if they have obtained the records - to upload the pet’s complete medical history. Multiple pets can be submitted within a single form, with separate histories for each.

Please note that this form is only for clients transferring their routine veterinary care to Animal Trust. For referral procedures where the pet’s ongoing care will return to the referring practice, please use the Referrals section of our website so the request can be processed correctly.
